Sick note for burnout

Whether a doctor can sign you off work for burnout, what the WHO and the NHS say it is, and how self-certifying, notes and sick pay work.

The short answer. Yes, a doctor can sign you off work if burnout is affecting your health. The World Health Organization classes burn-out as an occupational phenomenon, not a medical condition, so a note is based on how it is affecting you, such as exhaustion, low mood or anxiety. You can self-certify for the first 7 days. After that, a doctor decides whether you are fit for work.

Is burnout a medical condition?

No, not on its own. The WHO includes burn-out in its International Classification of Diseases (ICD-11) as an occupational phenomenon and states plainly that it is not classified as a medical condition. It describes burn-out as the result of chronic workplace stress that has not been successfully managed, marked by three things: exhaustion, mental distance from or cynicism about your job, and feeling less effective at work.

The NHS describes burnout as physical and emotional exhaustion, and says it may also increase your risk of anxiety and depression. Long-term stress can cause it, but the two are not the same: stress tends to feel like too much, while burnout can feel like you have nothing left to give. The signs the NHS lists include feeling tired and drained, losing motivation, withdrawing from others, struggling to get things done, and changes to your sleep or appetite.

Can you get a sick note for burnout?

Yes, where burnout is affecting your health. NHS fit notes can only be issued for a person’s own medical problems, and a private note from us is likewise about your health. So the doctor looks past the label. They assess what burnout is doing to you, the exhaustion, low mood, anxiety or stress, and whether that stops you working. Whether a note is right, and what it says, is their decision.

The NHS says that if burnout is affecting your daily life or mental health, you should talk to a GP or contact NHS talking therapies. You can refer yourself to talking therapies if you are 18 or over (16 or over in some areas).

If a GP appointment is hard to get, a mental health sick note from a GMC-registered UK doctor is another option. It is a private medical certificate, not an NHS fit note, and your employer decides whether to accept it; government guidance for employers says they can. It is not valid for Universal Credit or Employment and Support Allowance (ESA), and the doctor may decline if a note is not clinically appropriate.

Self-certifying for the first week

For an absence of 7 days or less you do not need a note at all, whatever the reason. You tell your employer you are not well enough to work. For statutory sick pay, your employer cannot require medical evidence for the first 7 days. If they ask for a doctor’s note sooner anyway, any fee should be covered by them. The self-certification guide explains the form some employers use.

If you need help now. If you feel you cannot keep yourself safe, or someone’s life is at risk, call 999 or go to A&E now. For urgent mental health help that is not an emergency, use 111 online or call 111 and choose the mental health option. Samaritans answer free on 116 123, day or night.

How long can you be signed off with burnout?

No NHS or government guidance sets a standard period off work for a person with burnout. The NHS says burnout can take time to recover from, and that it matters to deal with what is causing it, not just the symptoms. How long you need is for a doctor to assess. What the rules do fix is the length of each NHS fit note: up to 3 months at a time in the first 6 months of a condition, with further notes if needed.

Dealing with the cause at work

The NHS point about dealing with the cause matters most when the cause is your job. Your employer has a legal duty to assess the risks to your health from stress at work and share the results with you. A note that says you may be fit for work can suggest changes such as altered hours or amended duties, and a phased return lets you build back up over a period you agree. If your employer cannot make the changes, you are treated as not fit for work on the same note.

Pay while you are off

Since 6 April 2026, statutory sick pay has been paid from the first full day off sick if you qualify, for up to 28 weeks, at £123.25 a week or 80% of your average weekly earnings, whichever is lower. See how statutory sick pay works.

Burnout, stress and low mood

Burnout often overlaps with other problems. If stress is the main issue, read getting signed off with stress. If your mood has been low for a while, the NHS advice on depression applies. The time off by condition hub covers the rules that apply to every absence.

Common questions
